Hospital-to-Home Care
A Safe, Smooth Transition for a Faster Recovery
The first 48 hours after leaving a hospital or rehabilitation center are the most critical. Without the right support, the risk of a setback or hospital readmission is at its highest. At ForeverWell Home Care, we bridge the gap between clinical discharge and the comfort of home.
Serving families across Middlesex and Union Counties, we provide the vigilant eye and helping hand needed to ensure your recovery stays on track.
Why Transitional Care Matters
When a patient is discharged, they are often sent home with a complex list of instructions, new medications, and follow-up appointments.
Managing this while feeling weak or groggy is overwhelming. ForeverWell steps in to handle the logistics so you can focus on healing.
Our Comprehensive Transition Process
Discharge Coordination
We don’t wait until you get home to start caring for you.
Medication Reconciliation & Reminders
Confusion over new prescriptions is a leading cause of hospital readmissions.
Home Preparation & Safety
Before you arrive, we can ensure the home is ready:
Clear pathways for walkers or wheelchairs.
Live-In vs. Hourly Transition Care
Depending on the surgery or illness, you may need different levels of support:
| Service Level | Best For… |
| Short-Term Hourly | Minor procedures, biopsies, or outpatient surgeries. |
| 24/7 Overnight | Patients who may experience confusion or pain at night. |
| Live-In Transition | Major surgeries (Hip/Knee replacement), Heart bypass, or Stroke. |
